정부의 적극적인 정책적 개입과 홍보로 고혈압, 심근경색이나 뇌졸중 같은 질병에 대한 대국민인식이 이전과 달리 개선되었고, 이에 따라 예방과 치료효과가 향상되었다.
많은 사람이 LDL콜레스트롤이나 HDL 콜레스테롤을 자연스럽게 말할 수 있게 되었지만 여전히 '부정맥'이란 용어는 생소한 단어로 느껴질 수 있다.
특히, 최근에 부정맥에 대한 대국민인식조사에서 '부정맥'이 어떤 병인지를 아는 국민들이 10%내외였다는 것은 가히 충격적인 결과이기도 하다.
부정맥이란 무엇인가요?
부정맥이란 말 그대로 심장박동, 즉 맥박이 느리게 혹은 빠르게 불규칙하게 뛰는 현상이나 질병을 모두 아울러 지칭하는 말이다. 따라서 맥박이 느리게 뛰는 서맥도 있고, 빠르게 뛰는 빈맥, 한번씩 불규칙으로 뛰는 경우도 있다.
자다가 갑자기 사망하는 경우도 부정맥이 원인이다. 또한, 유전적요인에 의해서도 심각한 악성부정맥이 발생할 수 있다.
이와 같이 부정맥의 종류도 아주 다양하지만, 예후도 부정맥의 종류에 따라 천차만별이다. 부정맥은 환자에 따라 치료를 따로 받지 않거나 간단한 시술로도 치유할 수 있는 가벼운 경우도 있지만 제세동기의 삽입 등 지속적인 관심과 치료가 필요한 경우도 있다.
부정맥은 어떻게 느끼나요?
맥박이 느리고 빠른 정도에 따라 환자분들이 느끼는 증상이 다양하다. 다른 심장질환의 증상과 마찬가지로 숨이 차고 기운이 없을 수도 있고, 졸도를 할 수도 있다. 가슴이 두근거리면서 통증을 호소하는 분도 있다. 부정맥의 종류에 따라 증상이 전혀 없어 우연히 발견되는 수도 있지만 증상이 없더라도 처음 나타난 증상이 심장돌연사일수도 있다.
한마디로 부정맥은 콕! 찍어 ‘이러한 증상이 발생한다’ 라고 말할 수 있는 증상이 없다.

중풍을 부르는 부정맥 – 심방세동
심방세동이란 최근에 사회가 고령화되면서 빠르게 증가하고 있는 부정맥 중의 하나이다.
원래 심장박동은 좌심방의 특정부위에서 만드는 규칙적인 전기신호로 일정한 맥박을 유지하게 되나, 여러 원인으로 심장구조에 변형이 오면 하나이어야 할 전기발전장소가 여러 군데가 된다.
그 결과 심장박동이 불규칙해질 뿐 아니라 좌심방의 운동이 자루 속에 갇힌 여러 마리의 뱀처럼 꿈틀거리게 된다. 이로써 혈액순환에 장애가 생기고 혈전(피떡)이 잘 만들어지게 된다. 피떡이 만약 뇌로 날아가 혈관을 막게 된다면 바로 중풍이 발생할 수 있다.
뇌졸중(중풍)을 경험한 환자 5명중에 1명은 심방세동이 원인이다.
불행히도 심방세동이 있어도 스스로 알아 차릴 수 있는 증상이 없고 중풍을 겪고 나서 발견되기 때문에 예방측면에서 더욱 어렵다.
부정맥, 조기 진단이 중요하다
해당 전문 의료기관에서 심전도, 24시간 심전도, 운동부하검사 및 체내 삽입형기구 등의 다양한 방법을 이용하여 진단할 수 있다.
우리나라에서는 국가 암 조기검진체계의 구축(2004년), 생애전환기 건강진단(2007년) 등이 시행됨으로써, 전 세대와 계층을 아우르는 선진국에서도 찾아보기 힘든 포괄적인 건강검진체계를 갖춘 나라가 되었다고 하지만 검사항목 중에서 중요한 부정맥진단방법 중의 하나인 심전도측정이 빠져 있다.
심전도검사를 하지 않기 때문에 무증상의 부정맥을 미리 발견한다는 것은 아주 어려운 일이다. 심지어 60세가 지나 발병 후 에서야 처음 심전도를 검사해본다는 분도 있었다.
심전도검사가 국가가 시행하는 검사항목에 포함될 수 있도록 의료 관련 정책적 논의가 필요한 때이기도 하다.
