심장박동 장애, 혈액순환 곤란 -
◇ 심부전이란?
심부전이 생기면 혈액순환이 원활하지 못해 인체 각 조직 기능에 여러 장애가 나타난다.
심장의 가장 중요한 기능은 심근이 수축하여 혈액을 내보내고 전신을 순환시키는 펌프 작용을 하는 것이다.
심기능부전 또는 심장쇠약으로 불리는 심부전은 심장이 약해져 제 기능을 하지 못하는 경우를 일컫는다.
이런 심부전은 별개의 심장 질환이라기보다 모든 심장병으로 인해 생길 수 있으며, 모든 심장 질환의 결과라고도 할 수 있다.
예를 들어 심장 안에 있는 판막의 기능이 부실해도 심부전이 생기지만, 심근경색증을 심하게 앓고 난 후에도 심장근육의 기능이 나빠져 심부전 상태가 될 수 있다.
◇ 증상은?
심부전이 생기면 심장박동에 장애를 일으켜 심장에서 몸으로 보내는 혈액의 송출이 불충분하게 된다.
이 때문에 말초기관까지 가야하는 혈액순환이 원활하지 못하게 되면서 몸의 세포에서 일어나는 대사작용들, 즉 효소와 탄산가스의 교환이 충분치 못하게 되고 이에 따라 대부분 인체 조직기관의 기능이 약해지면서 여러 가지 장애가 일어난다.
뇌에서는 산소부족 때문에 실신발작이 일어나고, 폐에서는 호흡곤란이 일어나기 쉽다. 심부전의 가장 흔한 증상이 호흡곤란인 것도 이런 연유 때문이다.
처음엔 운동같이 몸을 심하게 움직일 때만 호흡곤란이 일어나다가 병이 진행됨에 따라 가만히 있어도 생겨날 수 있다.
심장으로 들어오는 혈액량의 차이로 누우면 더 호흡곤란이 심해지기도 한다. 그래서 자다가도 숨이 차 잠을 깨는 증상이 나타날 수도 있다.
또 다른 대표적인 증상은 부종이다. 이는 신장으로 흘러들어야 할 혈액량이 줄어들었기 때문이다. 신기능이 장애를 입으면 혈액이 통과하는 사구체의 여과량도 줄면서 자연히 수분이나 염분의 배설이 되지 않는다.
따라서 수분이나 염분은 다시 혈액 중에 축적되면서 모세관에서 말초조직의 세포 안으로 흘러들어 몸이 붓게 되는 것이다.
또 심장으로 영양을 공급하는 관상동맥의 혈류도 감소해 협심증과 함께 심장 비대나 확대가 있을 수 있다.
모든 심장 질환에서와 마찬가지로 심부전이 있어도 부정맥이 발생할 수 있는데, 이럴 때 생기는 부정맥은 치명적일 수 있다.
◇ 심부전의 종류
심근이 급성으로 손상을 입어 약해진 경우 보통 좌심부전으로 나타난다. 폐혈관 안의 혈액이 좌심으로 들어가지 못해 정체가 생기면서 숨이 차고 기침, 천식 같은 증상을 보이는 급성 심부전은 입술이나 손끝이 파랗게 되며, 식은땀이 나고, 불안과 초조한 심리상태를 보인다.
만성 심부전은 고혈압, 심근증, 선천성 심 질환 등이 원인인 좌심부전과 좌심부전으로 인해 폐울혈과 폐혈압이 높아져 우심에 부담을 줘 생기는 우심부전으로 구별된다.
호흡곤란이 주증상이지만 특히 우심부전인 경우 하지부종과 함께 간이 붓고, 복수가 차며, 소화 장애, 전신 쇠약감 등을 동반하기도 한다.
◇ 치료는 이렇게
심장의 부담을 줄이기 위해 절대 안정이 필요하며, 과로를 피해야 한다. 식사요법으로는 저염식이 요구된다.
심근의 수축력을 증가시키고 심장박동수를 줄여주는 효과가 있는 디지털리스제, 부종의 치료를 위해 이뇨제를 병행하기도 한다.
급성 심부전인 경우는 혈관을 확장시키기 위해 혈관확장제를 투여하기도 한다.
