관상동맥이란 심장에 혈액을 공급하는 동맥을 말하며 동맥의 안쪽에 지방질이 침착되고 비정상혈관근육세포 및 세포질이 증식되는 현상을 동맥경화라 한다.
맥경화증 환자들에게서 이른 아침, 흥분, 스트레스, 흡연 등의 물리적, 화학적 요인들이 함께 작용하여 동맥경화반이 파열되면 관상동맥이 완전히 막히게 되는데 이러한 질병이 급성심근경색증이며, 급성으로 치료하지 않으면 사망에까지 이르게 되는 무서운 병이다.
관상동맥질환이란?
심장은 혈관계를 통해 신체의 각 기관에 혈액을 전달하기 위한 펌프 작용을 하는데, 이러한 심장에 혈액을 공급하는 동맥은 3개가 있으며 이러한 동맥들을 관상동맥이라 한다.
리고, 심장에 혈액을 공급하는 관상동맥이 막히거나 좁아지는 질환을 관상동맥 질환이라 하며, 동맥경화증이 대부분의 원인으로 작용한다.
관상동맥 질환의 원인
동맥경화란 동맥의 안쪽에 지방질이 침착 되고 비정상 혈관 근육세포 및 세포질의 증식되는 현상으로 나이가 들면서 증가할 수 있으며, 동맥 경화증의 시초는 혈관 내피세포의 손상이 원인이다.
혈관 내피세포는 혈관 내막을 둘러싸고 있는 단일 층의 세포들이고 정상 내피세포는 동맥경화의 발생을 억제하고 혈전 형성을 억제하는 기능을 가지고 있다. 하지만, 어떤 물리적, 화확적 요인에 의해 내피 세포에 손상이 발생하면 동맥 경화 발생의 시초가 되는 것이다. 이러한 혈관 내피 세포 손상의 물리적인 요인으로 대표적인 것이 고혈압이고 화학적인 요인으로는 높은 저밀도 지단백 콜레스테롤 혈증, 낮은 고밀도 지단백 콜레스테롤혈증, 흡연, 높은 중성지방, 당뇨병 등이 있다.
이러한 요인들 모두가 내피 세포의 손상을 가져오고 동맥 경화를 유발하며 관상동맥에 작용하여 급사를 일으킬 수 있는 급성 심근경색이나 협심증, 머리 혈관에 작용하여 뇌경색, 하지 혈관 등에 작용할 경우에 말초혈관 질환 등을 야기 시킨다.
협심증과 심근경색
협심증은 관상동맥의 동맥 경화로 인하여 심장으로의 혈액 공급이 필요한 양보다 불충분할 때 일어난다. 혈관내경이 70% 이상 좁아지면 운동 혹은 흥분 시에 협심증 증상이 나타나기 시작한다.
협심증의 증상은 대부분 뻐근하고 조이는 듯한 흉통으로 나타나며 흉골 뒤에서 시작하여 팔, 목, 턱으로 전이되어 나타나기도 한다. 가역적이므로 안정을 취하거나 니트로글리세린 같은 약물에 의해 통증이 사라진다.
허혈성 심질환은 폐쇄의 정도와 시간에 따라 나타나는 일련의 연속적인 변화이며 협심증까지는 가역적이나 심근경색증으로 넘어가게되면 비가역적으로 심장근육이 손상된다. 이러한 동맥 경화증 환자들에게서 이른 아침, 흥분, 스트레스, 흡연등의 물리적, 화학적 요인들이 함께 작용하여 동맥 경화반이 파열이 되면 급성으로 관상동맥이 완전히 막히게 된다. 이러한 질병이 심근경색증이며 급성으로 치료하지 않게 되면 사망에까지 이르게 되는 무서운 병이다.
관상동맥 질환의 진단
관상동맥 질환의 진단은 흉통의 발생등 병력 청취가 가장 중요하다. 통증은 보통 신체적인 활동이나 스트레스와 연관되어 나타나지만, 연세가 많은 노인이나 당뇨를 가진 환자에서는 15-20% 가량에서 통증을 동반하지 않고 나타날 수 있다.
보통 지속시간은 15분을 넘지 않지만, 30분 이상 지속되거나 니트로글리세린에 반응이 없는 흉통인 경우 심근경색을 의심할 수 있어 빠른 시간내에 가까운 병원이나 응급실을 찾도록 한다. 심전도는 가장 기본적인 심장상태를 알 수 있는 검사로서 심근 허혈 부위를 평가할 수 있으나, 임상 소견을 고려하여야 하고 실제 심근의 허혈이 있어도 휴식 안정기에서 심전도가 정상인 경우가 많다. 조절돼 부하를 주어 심근의 허혈 여부를 평가하는 방법으로 운동부하 심전도 검사와 약물로 부하를 주어 검사하는 도뷰타민 스테레스 심초음파법이나 심장 핵의학 검사 등도 있다. 심장 핵의학 검사는 심각하게 혈류의 감소가 있는 심근부위를 방사성 동위 원소의 심근에 대한 관류 상태를 확인함으로써 허혈 부위를 밝혀내는 검사로서 유용하다.
심초음파 검사는 심각하게 혈류의 감소가 있는 심근 부위를 심근 운동 관찰을 통하여 알아내는 방법이며, 관상동맥 혈관 조영술은 심장혈관을 직접 확인할 수 있는 검사법으로 관상동맥 질환의 진단에 있어서 해부학적 진단의 기준이 되는 검사이며 중재적 시술이나 수술치료의 기초가 되는 검사이다. 보통 서혜부의 대퇴동맥을 통하여 시술하나 본원에서는 팔의 요골동맥을 통하여 오전에 입원하여 검사를 마치고 지혈 후 오후에 퇴원하는 시스템도 갖추고 있다.
관상동맥 질환의 치료
관상동맥의 치료에 정확한 진단이 필수적이며, 5분이상 지속되는 통증은 반드시 검사를 받아 보아야 한다. 위험 인자의 조절은 기본으로 금연하고, 체중조절 및 고지혈증을 치료하며, 혈압 당뇨를 잘 조절하고, 식이 습관을 잘 조절하도록 한다. 규칙적인 운동이 추천되며 일정한 힘을 일정기간 동안 쓰는 등장성, 호기성 운동인, 걷기나 수영등이 좋다. 하지만, 많은 힘을 순간적으로 쓰는 역기들기 같은 운동은 좋지 않다.
스트레스는 아드레날린 분비를 촉진하여 심장을 더 빨리 힘들게 뛰도록 만들고 혈관을 수축시키므로 스트레스의 원인을 파악하여 없애고 긍정적인 사고방식을 가지도록 한다. 필요시에 혈관 확장제나 혈소판 응집 제제를 사용토록 하고 협착 부위를 확장시키는 풍선 도자 확장술 및 스텐트 삽입술등의 비수술적 방법으로 대부분 치료가 가능하나 일부 환자에서는 관상동맥우회성형술과 같은 수술적 치료가 필요하다.
맺음말
관상동맥 질환은 완치되는 질환이 아니라 고혈압이나 당뇨병 같이 지속적인 관리를 해야 하는 질환이다. 혈전형성 억제 등의 약물 치료와, 규칙적인 운동 및 생활 관리 및 치료등 을 동반하여야만 치료될 수 있다.
